Statement of Faith
WE BELIEVE:
One Holy God
Eternally existent in three persons: the Father, Son and Holy Spirit.
Jesus Christ
Virgin-born Son of God. Crucified to take upon Himself the punishment for the sin of all mankind. Arose from the dead, thereby conquering sin, death and the grave. Jesus will personally return to the earth in power and glory.
Holy Spirit
Indwells those who receive Christ and empowers believers to live righteous and holy lives. We believe in the Baptism of the Holy Spirit along with its evidences, and the Gifts of the Spirit are available to believers today.
The Holy Bible
The inspired, infallible, authoritative source of Christian belief and teaching.
Man
Created in the image of God but fallen, lost and powerless to save himself.
Man’s Only Hope
Believe in Jesus Christ as Savior and Lord.
What We Celebrate
Water Baptism and Holy Communion are among the ways we celebrate our hope in Jesus Christ.
Healing
Consistent with His earthly ministry, Jesus Christ still offers healing for the physical, relational and emotional pain in our lives.
Lifestyle
When a life is changed by the love of Christ, we begin a journey of becoming like Christ. It is displayed inwardly with love, joy, and peace. It is displayed outwardly in lives of integrity and is shown relationally by love & grace.
Someday
Life, as we know it, will come to an end. Jesus will return and show Himself to be “King of Kings.” Everyone will be held accountable for the choices of their lives. A permanent, personal relationship with God, without the influence of evil and pain will be established for all who chose Christ. (Heaven)
The Church
The Body of Christ, of which Jesus Christ is the head, is comprised of all those who, through belief in Christ, have been spiritually changed by the personal work of the Holy Spirit. Believers are called to pursue God’s mission, both collectively and individually. For some it is a vocation (pastors, missionaries, etc.) for all it is a passion.

- I am committed to growing and maturing my relationship with God through Bible reading,
prayer, fasting, and active involvement both at church and in accountability relationships. - I am committed to developing the leadership gift on my life through consistent attendance at
team gatherings, events, trainings, and class time. - I am committed to making choices and living a lifestyle that is godly and above reproach.
- Knowing my life is an example of both Jesus Christ and Life School of Ministry, I will abstain from such
things as:
Dishonest gain, illicit and illegal drugs, indulgence in much wine or alcoholic beverages,
cohabitation, sexual immorality (sexual activity outside of marriage, including
pornography [“marriage” being defined as the union of one biological man and one
biological woman in a single, exclusive union, as delineated in Scripture (Genesis 2:18-
25)]).
I will abstain from sinful behaviors that would cause Christ to grieve and others to
stumble. - I will faithfully fulfill the responsibilities assigned through my school work and by the specific ministry I’m assigned to (if applicable).
- I am committed to Life School of Ministry. I know and passionately support the vision of the school and
am committed to seeing these purposes fulfilled to the glory of God. - I will support the leadership of Life School of Ministry by honoring the Pastors, staff, and Team Leaders
of the school and respecting other team members in both speech and action. - I am committed to working in a team environment, keeping a positive outlook, and exhibiting a
servant’s spirit at all times.
General Consent Statement
I certify that all statements that I made on my application are accurate and honest to the best of my knowledge.
I realize that my subsequent and continued enrollment to LSM depends on the accuracy of the information I have provided. I am also aware that any false information, misrepresentation or omission of facts regarding my character, or personal characteristics could potentially forfeit my enrollment to LSM. I also understand that any fees or tuition that I have paid is non-refundable, should this take place.
I consent to allow administrative leadership and LSM personnel to have access to all personal information including spiritual, medical, and psychological history. In addition, I will submit to all regulations of the School of Ministry and seek to maintain, by conduct and achievement, its high standards and beliefs.
I realize that my subsequent and continued enrollment to LSM depends on the accuracy of the information I have provided. I am also aware that any false information, misrepresentation or omission of facts regarding my character, or personal characteristics could potentially forfeit my enrollment to LSM. I also understand that any fees or tuition that I have paid is non-refundable, should this take place.

Background Consent Statement
During a students time at Life School of Ministry, they will be required to complete practicum hours. During the practicum hours they could consist of and not limited to serving in different ministries at Life Church, therefore a background will be run so that the student will be in compliance with Life Church guidelines to serve.
I hereby authorize Life School of Ministry and/or its agents to make an independent investigation of my background, references, character, past employment, education, credit history, criminal or police records, including those maintained by both public and private organizations, and all my public records for the purpose of confirming the information contained on my application and/or obtaining other information which may be material to my qualification for enrollment now and, if applicable, during the tenure of my education at Life School of Ministry.
I release LSM and/or its agents and any person or entity, which provides information pursuant to this authorization, from any liabilities, claims or lawsuits in regards to the information obtained from any and all of the above referenced sources used.
